The Opportunity
OrthoCarolina is a leading independent orthopedics practice, offering comprehensive musculoskeletal care. The practice’s 300-plus practitioners conduct more than one million patient visits annually across North and South Carolina. The physician-owned company is dedicated to providing innovative quality care to its community.
A culture of innovation drew the SVP of Technology Services Neil Stein to OrthoCarolina. Driving real change takes more than vision—it demands a willingness to challenge the status quo and the ability to execute. Neil and his team had both. What they needed was the right partner to help bring it to life.

The Strategic Approach

He put Presidio to the test. Early projects proved Presidio wasn’t just a vendor — it was a strategic ally, ready to lead OrthoCarolina’s innovation journey.
A core focus of that journey: cost avoidance for both the organization and its patients. By shifting surgeries from costly hospitals to surgical centers —and smaller procedures into OrthoCarolina clinics—the team aims to lower expenses while delivering more personalized, local care.
To make this vision a reality, OrthoCarolina streamlined its operations, divesting from Physical Therapy and MRI branches to concentrate on its clinics. From there, it zeroed in on modernizing its technology. Outdated systems were draining resources and distracting from strategic goals.
Presidio stepped up, partnering with Microsoft to move OrthoCarolina’s critical infrastructure to Azure, with plans to migrate the full data center. This shift boosts agility, scalability, and cost control—while Presidio handles the backend complexity.
“Just like our physicians should practice at the top of their license, our tech team should focus on high-value work,” said Stein. “I could help on the help desk—but that’s not the best use of my time. Presidio can take that off our plate.”
What’s Next
With a solid cloud foundation and a trusted partner in Presidio, OrthoCarolina’s technology services group is ready to accelerate innovation. Next up: modernizing applications and infrastructure to enhance patient experiences while cutting unnecessary costs.
OrthoCarolina and Presidio are also exploring new revenue opportunities, including offering the OrthoCarolina technology services platform to third-party clinics. These clinics could leverage a fully managed, cloud based solution—opening access to advanced capabilities without the overhead. It’s just one example of how OrthoCarolina’s forward thinking approach, in partnership with Presidio, is redefining orthopedic care
